MGHA 2008-09 Season New Player Application

Thanks for you interest in playing with the Madison Gay Hockey League (MGHA). We are currently taking applications for spots for the 2008-09 season that begins in September.

Please take a few minutes to tell us a little bit about yourself and why you'd like to skate with the MGHA. Fill out the application completely and answer all questions honestly and openly. Once we receive your completed application, the MGHA Recruiting Committee will contact you within seven days to set up a time to meet with you.
